Case management and/or social work experience with an MSW or BSW preferred. Duties include intake and assessment of new patients for basic needs, referrals to local community resources, and assisting patients in applying for government benefits such as Medical Assistance. Must have experience working with diverse populations and be able to commit to a set schedule (ex. weekly, or bi-weekly shift) during our clinic hours M-F 8:00 AM - 4:30 PM. We generally ask for a 4-hour-a-month minimum.
